We need your help in planning our activities for the coming year. Your time is valuable and we appreciate a few minutes of it in order to make this year the best yet. Thank you for your valuable opinion. We will hold a drawing of the returned surveys at our first meeting of the new year. Please complete this survey by the April 27, 2000 meeting.
